UPDATE A CLAIM

The court has requested additional information from each veteran who has previously filed a claim. Without this information, your claim cannot be processed. If you have any questions, or require assistance, please call our office at 1-800-229-7989. This is a 24-hour line. The deadline for filing is September 20, 2021. Please do not delay.

1. Identify each branch of the military that you served in and the dates of that service.

2. List each duty station you served at between 2000-Present, along with the dates of service for each one.

3. Provide each MOS (Military Occupational Specialty) you held during service between 2000-Present, along with the dates of service for each one.

4. Please verify if you ever used Combat Arms Earplugs version 2 (CAEv2): Yes No

5. Please indicate if you used CAEv2 earplugs in combat: Yes No

6. Did you ever use the CAEv2 earplugs, the Indoor-Outdoor Range Earplug or the EAR ARC earplug as a civilian and under what circumstances?

7. Identify the extent of any hearing loss and/or tinnitus sustained from your use of the CAEv2 earplugs:

8. When (Year) did you first notice that the CAEv2 earplug was not providing adequate protection from loud noises?

9. Have you had your hearing tested? If so, when, and what was the diagnosis?

10. Have you received disability benefits as a result of hearing loss or tinnitus? What is the source of those benefits?
